Buckley Amendment Consent Form

The Buckley Amendment Consent Form is a document signed by student-athletes each year as a part of the NCAA Student-Athlete Statement.  The Buckley Amendment, also known as the Family Educational Right and Privacy Act of 1974 (FERPA) is federal legislation that was enacted to protect the privacy of a student’s educational records. As a condition of eligibility, student-athletes are asked to sign the Buckley Amendment Consent Form to grant its institution the right to share portions of their educational record to their conference, the NCAA, or other entities deemed necessary to evaluate or determine the student’s eligibility status.
